How do I schedule an appointment?

Our office is paperless. Please click below to complete the appropriate electronic registration form:

New Patients (Age 0-12 Months) / Registro de Pacientes Nuevos (0-12 meses de edad)

New Patients (Age 13 months -20 years) / Registro de Pacientes Nuevos (de 13 meses a 20 años)

Upon receipt of your registration, our New Patient Coordinator will reach out to you to schedule your appointment (typically the same business day). If you have any questions or if you need assistance, please call our office at 979.353.0006.

If your first scheduled appointment is a Well Visit and your child has any known, complex medical issues (such as developmental, chronic diseases, mental health, behavioral, ADHD, etc.), please let our Coordinator know when scheduling- we will block some extra time for your first appointment to ensure that all of your concerns are addressed.

If you are transferring from another practice, we will need your immunization records prior to your first visit. If you need help requesting those from a previous doctor, let our Coordinator know. We are happy to assist.

Do you accept my insurance?

We are in-network with Cigna, BCBS, Aetna, United, Humana, Ambetter, Community Health Choice, Amerigroup, Tx Children’s, and Medicaid.

What if I do not have insurance?

If you have different coverage, we will help complete the paperwork you need to file your claim. If you are self-pay, we offer a cash discount due at time of service. Payment in full is expected at the time of your visit but we offer discounted rates for cash-pay patients.  We also participate in the federally-funded “Vaccines For Children” (VFC) program,  which helps children get the vaccines they need if families are uninsured or underinsured.
